CLEO Systems is delighted to announce that Integrated Care 24 (IC24) and NHS Norfolk and Waveney Integrated Care Board (ICB) have been awarded Best Contribution to the Improvement of Urgent and Emergency Care at the Health Service Journal (HSJ) Partnership Awards 2025.
This national recognition celebrates the outstanding work behind the Unscheduled Care Coordination Hub (UCCH) – an innovative service underpinned by CLEO CORE, our advanced patient management system.
Based at Reed House in Norwich, the UCCH is transforming the delivery of Urgent Care in the region. The hub brings together clinicians from across health and care services to collaboratively manage ambulance waiting lists, assess referrals from paramedics, and direct patients to the most appropriate care pathway. This joined-up approach often helps patients avoid unnecessary hospital admissions, improving outcomes and relieving pressure on emergency departments.
CLEO CORE is at the heart of the UCCH’s success. The system enables real-time clinical triage, seamless care coordination, and safer, more efficient patient journeys.
